Dental Patient Forms

We ask that all of our new patients please take a moment to fill out our patient forms, which can be accessed online using the link below. By doing this ahead of your visit, we’ll be able to input your information into our system before you and your child arrive at the office. Dental Insurance & Medicaid Welcome!

Not only are we in-network with most major PPO dental insurance plans, but we also accept Medicaid. Our team will file claims on your behalf to maximize your benefits at each visit, and we’ll submit benefits checks with your insurance company to keep you informed of any out-of-pocket expenses you’ll be responsible for. If you have any questions about your child’s insurance coverage or need help navigating your benefits, our team would be happy to assist you.

In-House Dental Membership Plan

If your child is uninsured, we would recommend taking advantage of our in-house membership plan. Instead of dealing with the various barriers that dental insurance presents, you can receive coverage for all of your child’s basic dental care, as well as discounts on many other treatments. All you have to do is sign up in-office and pay a low annual fee (much less compared to the average insurance premium). With us, there are no waiting periods, deductibles, annual maximums, or confusion about what’s covered and what’s not!
